Consumer Direct Care Network New Mexico has been providing in-home care services and supports to New Mexicans like you since 2002. We help people direct their care and live the life they want in their homes and communities.

We're with you as you manage your in-home care.

Our services and supports help older adults and people of all ages with disabilities and other long-term care needs remain safe and independent at home. We work alongside individuals and families to help them understand, enroll in, and manage care services, providing guidance every step of the way.

When it comes to your health and happiness, we are always here for you.

Our services are available statewide, with Spanish and Navajo-speaking staff available to support participants and their families.

I need to submit Sick Leave.

Caregivers must submit complete the Paid Sick Leave Request form then email it to the CDNM team.

I need to change my status.

Caregivers can submit their completed Change of Status form to the CDNM team.

I need to set up my payments.

Caregivers can submit the Pay Selection Form to the CDNM team.

I want a pay stub or W2.
