Consumer Direct Care Network New Mexico (CDNM) offers Support Broker Services to help older adults and people with disabilities take an active role in managing their home and community-based services through the Самостоятельная работа Community Benefit program.
You stay in control of your services — and your Support Broker helps you understand your options and carry out your plan.
What Does a Support Broker Do?
Your Support Broker works alongside you to:
- Understand your service plan
- Help you find and hire caregivers
- Stay within your approved budget
- Complete required paperwork
- Review and submit timesheets
- Adjust services as your needs change
They can also support you with:
- Housing-related goals
- Job application assistance and interview preparation
- Planning for independent living activities
Support can be provided on a short-term or ongoing basis, depending on your needs.
How It Works?
You choose the services you need based on your Comprehensive Needs Assessment (CNA), which is provided by your Managed Care Organization (MCO).
Once you hire a caregiver, your Financial Management Agent (FMA) works with you to:
- Complete hiring paperwork
- Process payroll
- Pay employment-related taxes
Your Support Broker helps you understand each step so you can make informed decisions about your care.
